consult our online Guide.

Although the Guide refers to General Donations, all the instructions are applicable to memorial donations.

Please remember to click the “Designated” check box if you are donating in memory of a specific person, and to use the Comments box at the bottom of the form to provide the name(s) of the person(s) being honoured as well as the fund to which you wish to direct the donation (e.g. Cemetery, Altar Guild, etc.), if applicable.
